The Vidiians suffer from a virus called 'the Phage', which destroys their genetic codes and cell structures. The disaster began more than two thousand years ago and has by now disintegrated their society. Thousands die each day, but their immunology has not yet been able to come up with a cure for the phage. The virus, however, has proven very adaptive and resistant.

No less terrible and oppressive for the Vidiians is the fact that in order to survive they had to become organ hunters. They do try to retrieve organs from deceased beings, but often time is a scarce good and living prey closer at hand, cruel as this may be. Each Vidiian has a 'homato' assigned to him whose task it is to find organs, just like Dereth, who retrieved lungs from Neelix's body and implanted them into Motura's.

The Vidiians, who are organized in a sodality, are aware of their unethical crimes. But there once was a time when the Vidiians were a very civilized people respected and known throughout the quadrant as scientists and explorers and great artists. Now they are forced to ambush ships in order to harvest organs for their dying population.

In terms of medicine, the Vidiians are more advanced than the Federation, which is sufficently proven by an analysis of one of their scanners. This device is able to gather more information than a tricorder; when pointed at a being it collects all specifics about its anatomy, including its DNA sequence. It can also be used as a weapon to paralyze one's victims.

On stardate 48784.2 the Vidiians manage to get hold of B'Elanna Torres. Klingons, they found out, might be resistant to the phage virus. Chief Surgeon Sulan extracted B'Elanna's Klingon genome, transformed it into energy and materialized a 100% pure Klingon female, making use of a device called 'genotron'.

Furthermore, the Vidiians are able to create extremely efficient forcefields that are nearly impenetrable (such as for phaser fire) and also shield from view by creating illusions.
