Print this page and cut out the card

  1. Right-click on the card
  2. Highlight and left-click “Save Picture As"
  3. Save the “orgdonorcard.gif” to your hard drive
  4. Open from a graphics program and print from there

AFTER PRINTING OUT YOUR CARD:

  1. Cut out card just outside the line
  2. Fill in the blanks, specifying any special instructions
  3. Sign in the presence of two witnesses
  4. Protect the card from damage with laminate or clear contac paper
  5. Discuss your wishes with your family
