Dear Cheri:
I received your recent letter dated December 2. I reviewed your chart and noted that I saw you on January 3, 1997. My notes indicate we had a fairly lengthy discussion regarding treatment of fibroids, including hysterectomy, myomectomy, medical management and regulation with hormones. My notes also state I gave you several informative pamphlets to read regarding these options.
My notes also state that January 17, I called you again to follow up on some of the laboratory tests. At that time, you failed to notify me that you had "left my office in tears." I am very sorry you felt this way about the information that was given to you.
I am delighted that you have been satisfactorily treated; however, after review of your chart and given the same findings as I found at your exam in January of 1997, I would still recommend hysterectomy.
