Pharmaceutical Assitance Program
You will need W2's from the previous year to apply.

* you are disabled or are  age 65 or older, and

*live in Illinois at the time of filling out the claim, and

*Have an income level that in at or below the what is set for the year. You will need to check the Illinois Department of Aging web site for the current information. You will find the link below.

*file form IL-1363, Curcuit Breaker and Pharmaceutical Assitance Claim, postmarked on or before December 31.

Illinois now offers 3 programs for older people & people with disabilities to help pay for prescription drugs.

1) Illinois Rx Buying Club
    * members enjoy discounts on all prescriptions at participating pharmacies and by mail order.
    * For people 65 & older & people with disabilities at any income
    * No limits. No caps. No income restrictions. No paperwork. $25 annual fee for administrative costs.

2) Illinois SeniorCare
    * A Medicaid waiver program with income limits
    * Covers most prescription drugs and many over the counter drugs if prescribed.
    * You must be a citizen or qualified non-resident.

3) Circuit Breaker/Pharmaceutical Assistance
    * A state program for low income people whose incomes are a bit too high for SeniorCare.
    * Covers prescriptions for Alzheimer's disease, arthritis, cancer, diabetes (including insulin, the syringes and needles), glaucoma, heart and blood pressure problems, lung diseases and smoking related illnesses, multiple sclerosis, osteoporosis and Parkinson's disease.
    * Applicants are also automatically placed in the Illinois Rx Buying Club for   coverage of additional drugs.
