Defeat
ultimate weapon, you’ll find it flying around on the world map
somewhere. Keep ramming into it with the highwind and follow it until it
stops above a town. Then battle it until it flies off then find it again
and keep hitting it then fight it again. Keep doing this until it stops
above Cosmo canyon, fight it for the last time and when you kill it
you’ll get Clouds ultimate weapon. And as a bonus you get access to
ancient forest where you can get some good stuff.

Midgar (second visit)
This ones easy to find,
You must have Barret in your team and you'll find it on the steps in a chest
on the way to fight Hojo. (It's slightly camouflaged so look closely)
Tifa
Wall market
Remember that
pointless place with that broken gun in the wall market. On disk 3
you'll have to find the Midgar key from Bone village, go back to the
wall market in Midgar and go to that room with the broken gun and guess
what it's fixed and it hands over Tifa's Ultimate weapon.

Cosmo Canyon
On disk 3 go and see Bugenhagen
in his lab. You'll find that he is very sick, after his speech he gives
you Red XIII ultimate weapon. (Red XIII must be in your team)
Vincent

Lucrecha's cave
With Vincent in your
party get in to the submarine and turn right (from when you are setting
off from Juno) and find an under water tunnel. When you get to the end of
the tunnel get out of your sub and go into the cave and speak to Lucrecha,
after Vincent and her have a little chat she'll give you Vincent ultimate
weapon.

Shinra HQ
On the second visit to
Midgar when your under ground and you have two different ways to go always
turn right. you will eventually find a dead end with a ladder, climb up it
and to in to the Shinra HQ. Go into the locker area and pick up the HP
shout.
Cid
Rocket town
On disk 3 go into Rocket
town and talk to the old guy outside the house in the middle. Keep talking
to him and keep talking about the rocket. He will eventually give you
Cid's ultimate weapon.
Yuffie
Wauti
Go into Wauti and into
the tower. As Yuffie defeat all of the opponents and finally Yuffie's Dad.
He will now give you Yuffie's ultimate weapon.
