Behavioral error:

Continually forgetting to call eye doctor and schedule an appointment

When/where:

Entire month of December, at home

Why this is an error:

An appointment was necessary, and the phone number was clearly posted where I could see it daily.

Processes:

Memory
Information was continually introduced into short-term memory but was always quickly replaced with other information. Because of this, the knowledge that I had to make an appointment never entered long-term memory.

Conditioning
The desired behavior was previously punished with a bad experience at the eye doctor. Thus, not making another appointment resulted in negative reinforcement (not having to go to the eye doctor, eliminating the chance of another bad experience).
