You then fill out the forms and email the file to the addresses in the instructions. Instructions are on the first page of the file. The Zone Registration form DOES NOT require a signature if you are submitting elecronically; however you MUST have a signed copy on file at your YMCA. By submitting this form electronically you are certifying that a signed copy is on file. |
