This Contract/Handbook is updated every six months as needed. By signing the Contract/Handbook & Financial Agreement, you are stating that you are in full agreement to the terms and conditions outlined in that form and below. This contract will cover a six month term from the date of signature. At which time you will sign a new Contract/ Handbook & Financial Agreement Form. If for any reason a new contract is not signed on or by the due date stated on, the old contract it will renew automatically every 30 days until a new contract is signed. Introduction: I would like to start by welcoming you into my home and Munchkins Playhouse. If you are viewing this handbook, then chances are you are considering placing your child/children in my care or you have just enrolled. I hope you will decide to make Munchkins Playhouse your home-away-from-home. I would like to start by saying Hello and introducing myself. My name is Shelley Drake and I am a mother of three, one son and two daughters. I am a professional childcare provider. I am also the owner and director of Munchkins Playhouse. I am trained in CPR and First Aid. In order to make our relationship as enjoyable as possible and to prevent any misunderstandings; I have created this handbook. While I realize it is quite lengthy, please read the handbook thoroughly. Every child enrolled at Munchkins Playhouse is recognized as a unique individual and will be encouraged to reach his/her full potential on an individual basis. Your child’s individual needs, abilities and learning style will be considered when planning all activities. We do not follow a strict regiment of scheduled day because we grab every opportunity to turn playtime into learning experience. I will provide balanced meals and snacks, outside playtime, and developmentally appropriate experiences for your child. Since play is one of the most important ways that children learn, play experiences will be an essential part of your child’s day. Your child will be treated with respect at all times. My goal here is to promote positive self-esteem, and to provide your child with the learning tools necessary to develop into an independent, self-sufficient, respectful adult. In order for me to concentrate on the important goals, it is necessary to establish a set of guidelines upon which my childcare program will be operated. Because your child is of utmost importance in your life, and because I take my role in their lives and my business very seriously, my handbook and contract are quite extensive, and outline nearly every aspect of your child’s care. Please carefully review all the enclosed information and feel free to discuss any of this information with me. As stated, this is a family home childcare business, which means it is also my home. Because I have my own family to consider, it is important that all of the parents respect my hours of operation, sick policy, and my need for prompt payment of weekly fees. I would hope that children and parents alike would respect my home and belongings the way you would respect your own. It is very important for us to maintain honest and open communication concerning your child’s well being and daily life. I am not a perfect person, but I do strive to do the best job possible in my chosen profession. Your comments and suggestions are all ways welcome, as they will offer me the opportunity to better myself and provide you and others with better service. Thank you for allowing, me the opportunity to care for your child. Let’s work together to keep your child in the type of environment we both desire for him or her. HOURS OF OPERATION: Full/part time care for “Munchkins Playhouse” begins MONDAY - FRIDAY at 5:00A.M. TO 4:00P.M. Drop off time will be no earlier than 5:00A.M. Pick up time is no later than the agreed upon scheduled time. LATE FEE WILL BE $10.00 PER HALF-HOUR PER CHILD BEGINNING AT YOUR SCHEDULED PICK UP TIME. UNLESS OTHER ARRANGEMENTS HAVE BEEN MADE No admittance after 8:30am unless prior arrangements have been made. It makes my mornings difficult to plan when I have to wait on children. ______________________________________________________________________________________________________________________ PREPARING YOUR CHILD FOR CHILDCARE Children, like adults, need time to adjust to new situations. Once you have decided to enroll your child in Munchkins Playhouse, here are some suggestions to help you prepare your child for change: * TALK ABOUT WHERE YOUR CHILD IS GOING AND WHY. * TELL YOUR CHILD ABOUT THE NEW PEOPLE HE OR SHE WILL MEET AND THINGS HE OR SHE CAN DO WITH THEM. * BRING YOUR CHILD FOR A VISIT TO MY HOME DURING PROGRAM HOURS BEFORE THE NEW ARRANGEMENT BEGINS. * DISCUSS WITH YOUR CHILD THE EXPECTATIONS AND RESTRICTIONS OF MY PROGRAM. IT IS ALWAYS BETTER TO DISCUSS THESE IN ADVANCE, SO YOUR CHILD KNOWS WHAT TO EXPECT FROM THE VERY BEGINNING. * ALLOW YOUR CHILD TO BRING ALONG A FAVORITE OBJECT (TOY, BLANKET, AND PHOTO) TO EASE THE TRANSITION. * WHEN YOU LEAVE, LET YOUR CHILD KNOW YOU ARE GOING, AND WHEN YOU WILL RETURN. ______________________________________________________________________________________________________________________ PICK UP AUTHORIZATION Both parents will have access to their child at any time unless court documents are provided. ______________________________________________________________________________________________________________________ ARRIVAL & DEPARTURE: Please do not allow your child to walk up to the door by themselves at drop off. It is important to take a minute to walk them to the door and greet me. This gives us both the opportunity to dicuss any schedule changes for the day or anything else. Children are to arrive clean and fed (unless arriving before listed mealtimes). Please bring your child dressed in weather appropriate clothing. If parents will be at a location that differs from the workplace (staying home for illness or an out of office meeting, for example), Please inform me when dropping off your child. Please make sure to have your child/ren remove their shoes upon arriving each day and place nicely under the stand so that no one trips on the shoes. It also helps to keep the snow, rain, grass and other soils from putting waer and tear on the floors and keeps everyones socks a little cleaner. It is normal for some children to have difficulty separating from parents or cry when being dropped off. Don’t sneak out while your child is not looking. This teaches your child that he/she cannot trust you. Be brief. Be firm. Be gone. A smile, a cheerful good-bye kiss, and a reassuring word that you wil be back are all that is needed. Most children are nearly always quick to get involved in play or activities as soon as parents are gone. I prefer that there are no drop offs or pick ups during the daily quiet time, but if this is necessary please be as quiet and as brief as possible. Children who arrive during quiet time will be expected to remain quiet until quiet time is over, so that others will not be disrupted from thier naps. PLEASE DO NOT RING THE DOORBELL! If your child needs to be picked up early for a doctor’s appointment, etc. Please call ahead so that I may have your child ready for you. THIS IS VERY IMPORTANT! : ) This is a time of testing, when two different authority figures are present (the parent and the provider). All children will test to see if the same rules apply. Do not rush out when they act up if we are talking, that reinforces that they can behave that way to make you leave when they want. By not rushing out, you reinforce respect for others and manners which I expect of them when they are here. During arrival and departure, I expect parents to back up my rules. Please be in control of your child during pick up times. This means if they are not in control of themselves and you ask them nicely to stop or be quiet 2 times. The third time I will expect you to give them a time out or do what is necessary to stop the behavior. Do not feel bad or embarrassed if I do this...it in fact shows them that they cannot test the waters. Also at pick up, please do not let your child leave the house until you are leaving also. They can be in the street in an instant or any other number of things could happen. IF YOU NEED TO DROP OFF OR PICK UP EARLIER THAN SCHEDULED, YOU MUST DISCUSS THIS WITH ME AND RECEIVE APPROVAL AT LEAST 48 HOURS IN ADVANCE. ADDITIONAL FEES MAY APPLY AND REMEMBER THAT I AM IN NO WAY OBLIGATED! ______________________________________________________________________________________________________________________ SIGN IN SHEET: Parents will be required to sign your child/ ren in and out each day they attend. Your sign in sheets are in a black binder with your last name on them. I keep them right by the door your pick up and drop off at. ______________________________________________________________________________________________________________________ VISITATION: My family childcare home has an “open door” policy. I encourage parents to make “brief” drop-in visits. However, I ask that courtesy be considered. I ask that these visits not be made between the hours of 11:30p.m. to 3:00p.m. As this is naptime. I ask that your presence does not disturb the harmony of the childcare environment. Also, keep in mind that I must continue to give the children my undivided attention. Open door policy does NOT mean that my door will be unlocked. I believe that is it extremely important to keep the doors locked for safety of the children. I would appreciate you taking into consideration my schedule when droping in or calling, and remember that visitors usually cause the children to react in an excited manner that does not normally occur when I am alone with the children. ______________________________________________________________________________________________________________________ REGISTRATION: The following forms must be completed and received before child care begins: 1.) Check List 2.) Acceptance & Financial Agreement Form 3.) Contract must be filled out and signed 4.) Child Health Form 5.) Emergency Medical Care Form 6.) Emergency Contact Form 7.) Transportation Permission for Outings Form 8.) Getting to know you better form 9.) Field Trip Permission Form The information on these forms must be kept current. It is important to keep in mind that, in compliance with state laws, these forms must be updated from time to time. If there is any change, the Parents do hereby agree that they will notify the Provider immediately. The Child’s Health Records must be updated on a yearly basis. ______________________________________________________________________________________________________________________ NON-DISCRIMINATION CLAUSE Every child is unique and special to Munchkins Playhouse. No child will be discriminated against because of race, age, color, national origin, religion or special need. ______________________________________________________________________________________________________________________ ENROLLMENT FEES/HOLDING FEES: 1 weeks fee has to be paid in full upon signing this form and will not be returned in the event that your child/children is/are placed in alternate childcare. For childcare spaces to be held longer than 2 weeks, then half of your regular weekly payment will be required each week your child is not yet in attendance. I will not hold any spaces open without receiving regular weekly payment. This fee will be credited to your account for following weeks after your first week of childcare. These fees are nonrefundable should you decide to withdraw your child before our agreed upon start ______________________________________________________________________________________________________________________ WEEKLY PAYMENT: Payments are to be made in cash. Your weekly payment reserves a space for your child in my childcare program, and is not based on days your child/ren attends. You are responsible for your weekly payment as long as your child remains enrolled at Munchkins Playhouse. No Discounts are given if your child does not attend. Payment should be made on last morning your child attends for that week. It will be considered late if payment is not received by pick-up time that day, there will be a $15.00 late fee applied. You may drop off the payment by 4:30pm that day to prevent the $15.00 daily late fee. If you choose to wait then the daily late fee of $15.00 will be applied the next morning. Then a $15.00 late fee per day your payment is late until it is paid in full. If your child/ren is/are not present the day payment is due then payment must be made by 2:30pm on that day to avoid late fees. If you do not make your payment by 2:30pm then late fees stated above apply. I reserve the right to deny childcare services for your child/ren until full payment is received in full. But please keep in mind that the weekend days also counted in as days late. If unpaid fees remain unpaid for 2 weeks, I will take the needed steps to collect your past due balance. Any fees incurred are the responsibility of the parent. If parents’ fail to pay amount owed for childcare in full, amount will be turned over to attorney with 8% interest on unpaid balance computed monthly plus attorney fee and court cost Also, keep in mind you will be required to pay your weekly payment when I take my vacation time. If you are planning to seek substitute childcare during this time, you will need to pay for their services, also. THE SUCCESS OF MY BUSINESS DEPENDS UPON YOUR PROMPT PAYMENT IN ORDER TO TAKE CARE OF DAY-TO-DAY EXPENSES THAT ARE ENCOUNTERED. The day to day business cost is 42% of the gross of the daycare. Another 20% goes to business fees, and business taxes. What is left, minus self-employment taxes, is the gross income of the provider. Your rate not only reflects my salary but the operating cost of the business. I hope this helps with understanding the rates and the reasoning behind my payment policies. ______________________________________________________________________________________________________________________ TRIAL PERIOD: . There is a two-week (10 days) trial period for each child; beginning the day your child actually begins care. It is normal for children to experience some separation anxiety and they need time to adjust to a new environment. My desire is to give your child time to make this adjustment before determining whether or not I can adequately meet your child’s needs along with the needs of all the other children in my care. This is to ensure harmony throughout the childcare home. It is important that we all have a good working relationship with total cooperation from children, parents and provider. Either party may terminate this agreement within this trial period; with at least 72 hours notice, with or without cause. Pre-paid fees will be NONREFUNDABLE if parents initiate the termination. ______________________________________________________________________________________________________________________ WITHDRAWAL: Parents may withdraw their child from Munchkins Playhouse by giving a two weeks notice IN WRITING. You must provide a written statement that includes the last date of care and the reasons for withdrawal. You are responsible for two weeks payment whether or not you give me a proper 2 weeks notice. If your child/ren is absences more than one week without notification or payment it will result in an automatic withdrawal. You will still be responsible for the two weeks of payment weather or not your child attends childcare. Re-admittance will include all back pay including late fees plus one week of childcare payment that will apply to your last week of care that your child/ren is/are in my care. Please keep in mind that the weekend days are also counted in as days late. If unpaid fees remain unpaid for 2 weeks, I will take the needed steps to collect your past due balance. Any fees incurred are the responsibility of the parent. If parent(s) fails to pay amount owed in full amount will be turned over to attorney with 8% interest on unpaid balance computed monthly plus attorney fee and court cost. ______________________________________________________________________________________________________________________ TERMINATION: I reserve the right to reevaluate any child or family’s enrolled in my childcare home to determine whether I can adequately meet the needs of the child and/or family. If I feel like our childcare arrangement are not providing what the child/family needs, or that it is not in the best interest of the other children/families in my childcare. I will provide the parent(s) with a termination notice. Two week’s notice will be given unless your child is an immediate danger to himself or the other children. OR the parent displays blatant disrespect and disregard for me, my policies, my childcare, etc. FAILURE TO ABIDE BY ANY OF THESE POLICIES MAY RESULT IN IMMEDIATE TERMINATION. ______________________________________________________________________________________________________________________ HOLIDAYS: On the following days Munchkins Playhouse will be closed for paid holidays: New Year's Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, the day after Thanksgiving, Week of Christmas, and New Years Eve ALTERNATIVE ARRANGEMENTS WILL NEED TO BE MADE ON THESE DAYS ______________________________________________________________________________________________________________________ VACATIONS & PERSONAL DAYS OFF: I reserve the right to have 2 weeks vacations at your weekly rate per week per calendar year and 10 paid sick/personal days per calendar year. You will be given at least a 2 week notice of these personal days, unless in an emergency. I will also give you at least a 3-week notice of my vacation time. It is the parent’s responsibility to obtain substitute care during these times, and to pay for their services. You are allowed up to 2 weeks vacation 1 week without pay and the other at your full weekly rate. These weeks must be taken as a full Monday through Friday weeks and cannot be split up with one day here and one day there. All unused weeks will not be carried over or credited. Vacation time may not be used towards 2 weeks notice for discontinuing care. During this time, your child/ren will not be permitted to attend daycare. If your child/ren do/does attend, it will not be considered vacation time and the full weekly rate will be charged. If you are in need for more than 2 weeks off, you will be required to pay your full weekly rate per week. You are required to give me a 3-weeks notice before your vacation, other leave. In the event that you do not give me a 3-weeks notice of vacation, other leaves, you will be required to pay for the weeks in full. I need to have advance notice in order to maintain my business expenses. Thank you for respecting this request ______________________________________________________________________________________________________________________ EXTENDED ABSENCE: In cases of your potential absence due to maternity or extended leave from your job, I will require half of your regular weekly tuition for the entire time of your absence to hold your child's position. You are required to give me a 3-week notice before your leave. If you fail to do so you will have to pay full rate for the time your child/ren are on leave. Unless it is emergency situation, at which time other arrangements will be made. ______________________________________________________________________________________________________________________ CHILD’S ABSENCES: Please inform me as soon as possible or by 8:00pm the night before your scheduled day if your child is going to be absent due illness or to arrival late or early departure. This will allow me to adequately prepare for the children’s meals and activities. I ask that you please use consideration. If you fail to inform me there is a $5.00 fee.. Payment in full is still due at the end of the week. If you child/ren misses their last day of the week then payment must be made by 2:oop.m that day. Unless it is emergency situation, at which time other arrangements will be made ______________________________________________________________________________________________________________________ NO CALL/ NO SHOW: If you do not call me before your scheduled day to tell me you do not need care for the day/days. It will be considered as a no call/ no show. There is a fee of $5.00 for each day your child does not attend with no call. On your Sixth no call/ no show you will be immediately terminated. (Please look at the termination section for further information). Unless it is emergency situation, at which time other arrangements will be made. ______________________________________________________________________________________________________________________ MEALS: I serve nutritionally balanced meals. Children are offered the meal, not forced to eat. They are encouraged to try new foods. Those who choose not to eat will not be served something else until the next scheduled meal or snack. All children will be served meals if they are present during the scheduled meal times. Meals and snacks provided by Munchkins Playhouse include: Breakfast - 7:00 a.m. ~ Lunch - 11:00 a.m. ~ Afternoon Snack - 3:30 p.m. Please if you enroll your child in a preschool program and expect me to prepare their meals they need to be here at the appropriate time or contact me to make arrangements. If you do not contact me to make prior arrangements, it will be your responsibility to feed your child the meal for on those days. * Milk, kool-aid, fruit juice and water are the ONLY beverages that will be served at Munchkins Playhouse. * If you child requires a modified diet due to recovery from illness, you will be required to supply the necessary food items. * If your child has any food allergies and/or not able to eat certain foods, a physician’s written note is required to be in the child’s file. * Sweets and treats will be kept to a minimum. They will be served during special occasions and holiday celebrations. * Please do not send gum, candy, or any other food items with your child unless prior agreement has been made. * If you enroll your child in a preschool program and expect me to prepare their meals they need to be here at the appropriate time. If you do not contact me to make prior arrangements, it will be your responsibility to feed your child the meal for on those days. IF YOUR CHILD IS GOING TO EAT MEALS HERE PLEASE HAVE THEM HERE BY THE FOLLOWING TIMES... BREAKFAST 7:00A.M. LUNCH 11:00A.M. SO I CAN COOK ACCORDINGLY TO FEED ALL THE CHILDREN. IF YOU ARE ARRIVING LATER PLEASE MAKE SURE YOUR CHILD HAS EATEN. PLEASE DO NOT HAVE YOU CHILD ARRIVE CHEWING GUM, EATING CANDY, OR OTHER FOOD AT ANY TIME. ALTHOUGH, TREATS BROUGHT FOR THE ENTIRE GROUP IS WELCOME. ______________________________________________________________________________________________________________________ NAPS/ QUIET TIME: Nap/quiet time is at 11:30-3:00 p.m. please have your children here before then. After lunch a rest period is required for older children. Babies or young toddlers who need more than this rest period will be allowed to nap as needed. I prefer that there are no drop offs or pick ups during the daily quiet time, but if this is necessary please be as quiet and as brief as possible. PLEASE DO NOT RING DOORBELL! ? CHILDREN WHO ARRIVE DURING QUIET TIME WILL BE EXPECTED TO REMAIN QUIET UNTIL QUIET TIME IS OVER, SO THAT OTHERS WILL NOT BE DISRUPTED FROM THEIR NAPS. ______________________________________________________________________________________________________________________ POLICY REVISIONS: Revisions to policies and procedures may be done with a minimum of 2 weeks notice UNLESS it is a new State or County regulation - these changes will take place immediately. Policies, contracts, and forms will be reviewed periodically and updated, if necessary. I will notify parents in writing of any changes. ______________________________________________________________________________________________________________________ TRANSPORTATION: I will take all possible precautions when transporting the children. I will ask that you leave your child’s car seat when transporting them is necessary. During any outing we take I will always have my cell phone on hand and will carry contact numbers with me in case of emergencies. ______________________________________________________________________________________________________________________ FIELD TRIPS: Children will participate in field trips at various times throughout the year. Parents’ approval must be given on the child’s contract/handbook And financial agreement form. Parent volunteers are always welcome and appreciated. Notice of field trips will usually be given at least 2 days prior. Occasionally, there may be fees related to the trips to cover entry fees, on-site refreshments, etc. ______________________________________________________________________________________________________________________ SUBSTITUTES: I may employ a substitute caregiver, if necessary. These caregivers will be trained in CPR & First Aid and be familiar with children before being left alone. They will be someone that I know and trust. ______________________________________________________________________________________________________________________ SMOKING IS PROHIBITED: NO SMOKING is allowed in my childcare home at any time. ______________________________________________________________________________________________________________________ SAFE ENVIRONMENT: I have a nice fenced in outdoor play area with lots of room to run and play. There also is a swing set for the children to enjoy themselves, fire extinguishers are kept up out of reach of children and checked annually, as well as smoke detectors. We practice fire drills and tornado drills monthly. Poisons and medications are kept out of reach. My carpet floors are cleaned monthly. I provide supervised play areas. This is to insure the safely and wellness for your child while they are in my care. ______________________________________________________________________________________________________________________ TORNADOES In the event of a tornado, we will go downstairs to the basement for shelter. Large comforters are stored there to cover the children for further protection. Along with other necessary items needed. ______________________________________________________________________________________________________________________ INCLEMENT WEATHER & UTILITIES OUTAGE In the event of severe weather, all efforts will be made to remain open and continue childcare services. However, a delayed opening, early closing, or full day closure will be at my discretion. IF A STATE OF EMERGENCY IS ISSUED, I WILL BE CLOSED. If we experience a power outage or water is turned off for city repairs for an extended period of time, it will become necessary to close Munchkins Playhouse for the day. ______________________________________________________________________________________________________________________ CLOTHING Clothing worn to childcare should be appropriate for the day’s weather and PLAY. Shoes should be closed toe and rubber-soled. Please do not send your child to childcare wearing any new or special clothing. Children are encouraged to participate in games and activities, and will have a more pleasant and enjoyable time if they are wearing comfortable clothing. Some activities may be messy. Please do not expect me to keep your child’s clothes clean and free from stains. If you need to go somewhere where your child needs to wear “good clothes”, let me know in advance. Bring the special clothes along, and I will be happy to change your child and have them ready at the end of the day. ______________________________________________________________________________________________________________________ CHILDREN’S PERSONAL POSSESSIONS Please don’t let your child bring any toy or other belongings to childcare. Do not bring your child with make-up or Chap Stick. Children tend to share these items. If Chap Stick is needed, it will be placed with other medications and applied as needed. ______________________________________________________________________________________________________________________ SUPPLIES Parents will provide as applicable, 1. Bottles 2. Children’s Tylenol or Children’s Motrin 3. Disposable diapers, Pull-ups, wipes, diaper rash ointment 4. Sun screen, insect repellant, Chap Stick 5. Personal hairbrush and comb (these cannot be shared) 6. Toothbrush and toothpaste. 7. 2 Extra changes of clothes for the appropriate seasonal outerwear 8. Child needs a small blanket or indoor sleeping bag and a small pillow for naptime. Spring: light jacket, hat, and 2 sets of clothing for the new season Summer: swimsuit, towel, and 2 sets of clothing for the new season Fall: light jacket, and 2 sets of clothing for the new season Winter: boots, snowsuit, mittens, hat, and 2 sets of clothing for the new season * PLEASE LABEL ALL ITEMS WITH A PERMANENT MAKER. You will find it easier to leave a full pack of diapers/pull-ups and wipes here, so that you do not need to carry a diaper bag back and forth every day. I will send a notice home when more supplies are needed. If it becomes necessary for me to purchase supplies and/or requested items for your child, you will be billed. You must bring 2 extra sets of clothing, including socks, must be left here at all times for all children. Please be sure to provide me with 2 new sets as seasons change and when your child outgrows the current sets. When I have to use your child’s extra set, I will wash the outfit they are currently wearing and that will then become their extra set of clothes. ______________________________________________________________________________________________________________________ TOILET TRAINING: I will be glad to assist you when your child is ready to enter the toilet training phase. I will not train your child for you! Toileting is another child developmental milestone, and as with all stages of development, it cannot be forced. It is a natural progression that will occur when the child is physically, emotionally, and developmentally ready. We need to be there to assist, NOT INSIST. Toileting skills are easily mastered once the CHILD decides she/he is ready to use the potty. If you are relaxed about the process, your child will be also. When your child has decided to fine-tune his/her toileting phase, I will request a continuous supply of Pull-ups to be used until your child has completed the process of toilet learning. Please do not bring your child in underwear until she/he has naptime and bedtime control established. During toilet learning, please dress your child in user-friendly clothing. Shorts and pants with elastic waists work best. Girls may wear dresses, if preferred. Please avoid tight clothing, pants/shorts with snaps and zippers, and overalls. YOU MUST PROVIDE 3 SETS OF EXTRA CLOTHING INCLUDING SOCKS AND SHOES AT THIS TIME. ______________________________________________________________________________________________________________________ ILLNESS AND SICK CARE A sick child should be allowed to recover fully after an illness so that other children in my home do not risk exposure, and so that your child is able to fully participate in childcare activities. If you are unable to remain at home with your sick child, it will be necessary for you to make substitute childcare arrangements at your own expense. If my family or I become ill due to exposure to a sick child, all of the daycare children may need to seek alternate care while my family recovers. This is disruptive to the children. Cooperation is extremely important on this issue. Obviously, it will not be possible to avoid the spread of all infectious illnesses. However, minimizing exposure and providing good hygienic procedures in my childcare home are the only means by which we can limit the problem and resulting inconvenience. Accordingly, THE FOLLOWING POLICIES WILL BE STRICTLY ENFORCED FOR THE BENEFIT OF ALL INVOLVED IN MY FAMILY CHILDCARE HOME: FEVER: A fever is a sign that the body is fighting some problem. The importance of a raised temperature depends on what is causing the fever. A temperature 100 degrees or higher means a child should stay home. In the event of this type of temperature, the child should not come to daycare until the temperature has been down for 24 hours without the aid of a fever reducing medication such as Tylenol. If your child wakes with a high temperature and you administer a fever reducer, this generally only lasts a few hours and I will have to call you at work to come pick up your child. In the meantime, the other children have possibly been exposed to an undiagnosed illness. VOMITING OR UPSET STOMACH: A child who has been vomiting can easily spread germs. If your child vomits while at daycare, you will be expected to come immediately to remove your child from daycare. If you are not able to come as soon as you are called, please arrange for someone else to come pick up your child. The child must stay home until 24 hours have passed with no vomiting episodes. DIARRHEA: When a child has a single loose stool, he/she does not need to be at home. However, if a child has very runny stools that cannot be contained in a diaper, or the child cannot reach the toilet in time, the stool may contaminate the daycare setting and this child must remain at home. Please use your discretion with this. If the child has diarrhea that's not contained, you will be called to come pick up your child from daycare. RUNNY NOSES: Children with constant runny noses that are not caused by allergies may spread germs everywhere. They may wipe their noses on their hands, then rub them on other children, toys and on surfaces. This is a difficult one to call. Please keep in mind how you would feel if another parents brought their child to care and exposed your healthy child. These cases will be handled on a case by case basis. LICE: I run a nit free childcare. If your child has any nits eggs or louse bugs you will have to take your child home and they may not return until they have been treated and all nits have been removed. I reserve the right to determine whether your child should remain in my childcare home when illness is a consideration. If your child displays any of the above symptoms while in care, I will remove your child from the group and notify you or an authorized adult to pick up your child. Parents/authorized adults have ONE HOUR from time of notification to pick up your child. NO EXCEPTIONS. Your child may return 24 hours after a temperature has returned to normal, 24 hours after your child is no longer vomiting, or 24 - 48 hours, depending upon the illness, after the first dose of antibiotic. In some cases, you will be required to submit a signed note from your child’s doctor before your child may return to childcare when he or she may be in danger of exposing someone else to an illness. CHILDREN ARE WELCOME WHEN THEY HAVE ONLY A MILD COLD (CLEAR, RUNNY NOSE) AND ARE ABLE TO PARTICIPATE IN THE DAY’S ACTIVITIES. ______________________________________________________________________________________________________________________ ALLERGIES Do be sure that you fill out completely the information pertaining to your child’s health status and list all allergies to pollens, funguses, bees, food, etc. If your child takes any medications whether or not they are taking while in my care, so we know what it is and what the side effect are (i.e., drowsiness, hyperactivity, diarrhea, stomach ache, etc.) so that I may watch for these symptoms in your child. ______________________________________________________________________________________________________________________ MEDICATIONS I must obtain written permission prior to administering any prescription medicine. The container should be clearly labeled with YOUR CHILD’S NAME, name of the medicine, dosage, administration frequency, and the name and telephone number of your child’s physician. The medicine must be in the original container as dispensed by the pharmacist. You may want to ask your pharmacist to divide it into 2 bottles so that I can keep one and you wouldn’t have to worry about forgetting it at home or here. All medication must be handed to me! Do not leave medication in your child’s bag--this is very dangerous! I cannot administer any medication to your child that he/she has never taken before. This is in case the child has a reaction to the medication. I must know if you have given any medication during the 24 hours prior to coming to childcare. If there is an emergency, I need to inform the doctor of all medications taken. This will also prevent overdosing in the event that your child is very ill! ______________________________________________________________________________________________________________________ INJURIES Every attempt will be made to ensure that your child/ren will be in a safe environment. But, no matter how much we monitor conditions, accidents do occur. Bumps, scratches and scrapes are inevitable when children play. Minor cuts and abrasions will be treated with soap and water and properly bandaged. In the event of a serious accident, you will be contacted for instructions. If you or your emergency contact person is not available, emergency actions and/or whatever means is necessary will be taken in order to provide the best care for the health of your child. YOU OR YOUR FAMILY INSURANCE IS RESPONSIBLE FOR THE COST OF MEDICAL HELP OR TREATMENT DUE TO ACCIDENTS OR ILLNESS WHILE IN CHILDCARE AT MUNCHKINS PLAYHOUSE. In case of a major emergency, I will attend to your child first, call 911, and then notify you as soon as possible. If a parent is not available, other family or friends on your enrollment form will be notified. IT IS INCUMBENT UPON YOU TO CHECK WITH THE HOSPITAL EMERGENCY ROOM TO FIND OUT WHAT KIND OF FORMS ARE REQUIRED TO BE ON FILE FOR TREATMENT IN CASE OF PARENTAL ABSENCE ______________________________________________________________________________________________________________________ PLAY TIME: There is time each morning and every afternoon for plenty of free play both indoor and outdoor when weather permits. The TV is on for cartoons before 9:00am and 30 minutes before naps for those who choose to watch cartoons. At 3:00p.m, we might watch a movie that the children will take turns choosing. ______________________________________________________________________________________________________________________ OUTDOOR PLAYTIME: Children will play outdoors as the weather permits. It is important for the children to get fresh air, explore, and run and play. ______________________________________________________________________________________________________________________ OTHER ACTIVITIES: Include painting, coloring, play dough, puzzles, games, swings, slide, plus much more. ______________________________________________________________________________________________________________________ HOUSE RULES Please respect my profession, my home, my family, and me. The respect that you show me, including my home, furnishings, equipment, yard, and other children will communicate itself to your child and make for a better working relationship. There are certain house rules that all children will be taught and expected to follow. This is for the safety and well being of everyone. * No running permitted in the house * Foul language back talking or yelling will not be tolerated * No teasing or name calling * No hitting kicking, pushing, pinching, biting or hair pulling * No standing or climbing on chairs, tables, or on any other furniture * No wrestling * No throwing or intentionally breaking anything * Drinks are only for designated areas * Children are not permitted to lift and/or carry other children while in my house or on my property * Children are not allowed to play outdoors unsupervised * No pulling out grass from the lawn or throwing rocks, sand or toys * No gum or candy is allowed _____________________________________________________________________________________________________________________ DISCIPLINE POLICY According to each child's individual needs, behavior management or discipline techniques are as follows: 1.) Positive reinforcement- Acknowledge child's good behavior. 2.) Redirect - Turn child’s attention in another direction. 3.) Give options- "would you like to put away the toys?" 4.) Now and Then - Now it's time to pick up, then we can go outside 5.) Time out -used only as a last resort, and only when child is being harmful to self, others, or equipment. ______________________________________________________________________________________________________________________ DAMAGES: If your child causes any damage to my home or its contents, you will be held 100% liable for the replacement costs. ONCE THE CHILD’S PARENTS ARRIVE AT MY CHILDCARE HOME, THE PARENTS ARE SOLELY RESPONSIBLE FOR THE CHILD’S ACTIONS (E.G., IF THE PARENT GIVES PERMISSION FOR THE CHILD TO GO OUTSIDE ALONE, WHATEVER HAPPENS WHILE THE CHILD IS OUTSIDE IS THE RESPONSIBILITY OF THE PARENT). ______________________________________________________________________________________________________________________ OUR MUTUAL RESPONSIBILITIES: * Treat each other and the children with respect. * Plan together for a consistent program of toilet training at an appropriate age. * Agree to feel free to discuss any suggestions, concerns, or ideas that will benefit this childcare. * I operate my childcare in a “family-type” atmosphere. We work together to raise healthy, happy, well-adjusted children. Sharing is Caring..... ______________________________________________________________________________________________________________________ PARENT/PROVIDER COMMUNICATION: * Open communication is extremely important to me and beneficial to the well being of your child. Upon enrollment, I encourage you to share any questions or concerns you may have concerning the care of your child. It is important that we share a similar childcare philosophy between us. Please be upfront about your expectations so that we may work together to achieve these goals for your child. * I welcome questions, comments, and discussions that affect a positive outcome for your child. Sensitive issues will be discussed outside of regular hours either by phone or a scheduled conference. * If you call during the day, please be aware that I may be busy with the children and may not be able to answer the phone. If you will leave a message on my voice mail, I will return your call as soon as possible. The best time to call during the day is usually between 12:30pm & 2:30pm while children are napping. * You may email me anytime at Shelley@munchkinsplayhouse.com. I check my email periodically throughout the day, and will reply usually during naptime. I would also like to have your email address in order to communicate with you. * Families frequently have changes - new homes, new jobs, and new telephone numbers. Please keep me informed. Accurate information is critical for efficient, professional handling of emergencies. ______________________________________________________________________________________________________________________ PARENT RESPONSIBILITY: * The person that is picking up the child/children must come in and have visual contact with the provider. Make sure that if someone other than yourself is to pick up your child that I have a note from you in writing or a phone call, prior to the drop off/pick up. * Bring your child well rested, clean/bathed, and appropriately dressed for indoor and outdoor play. * Pick up your child at the scheduled time, if you are going to be late, please call. * Take an interest in our program and talk with your child about the activities at childcare. * Notify me as soon as possible, if your child will not be coming for a scheduled day. * Discuss the house rules with your child and respect them. * Arrange for a back-up caregiver in the event that I am unable to provide care on a given day. * Notify my immediately of any contagious illnesses. (See illness and medication policy)] * A clean change of clothes will be kept in the childcare home. * Adequate supply of diapers, wipes, diaper rash cream, and daily supply of infant formula will be provided to childcare site if needed. * Report change of phone number, address, employment, work hours, vacation time, as well as any changes in child health, or family status. * You are required to give me a 3 week notice of vacation, other leave, and 2 week for termination. Payment for the 2 week notice period is due whether or not the child is brought to my childcare. You are allowed 1 weeks vacation without pay. If you want more than 1 week off, you will be require to pay half of the weekly rate per week. In the event that you do not give me a 3-week notice of vacation, other leave, you will be required to pay for your vacation week in full. I need to have advance notice in order to maintain my business expenses. Thank you for respecting this request. * Any fees incurred are the responsibility of the parent. If parent’s fails to pay amount owed for child care in full, amount will be turned over to attorney with 8% interest on unpaid balance computed monthly plus attorney fee and court cost. • PLEASE TREAT MY FAMILY AND HOME WITH RESPECT. WE ARE JUST A NORMAL FAMILY LIKE YOURS. WE EACH NEED OUR TIME AND SPACE AWAY FROM CHILDCARE. • PLEASE REMEMBER THAT THERE ARE DAYS WERE WE HAVE PLACES TO GO OR APPOINTMENTS THAT WE HAVE TO GO TO AT THE END OF THE DAY. FOR THIS REASON PLEASE DON’T BE LATE. CHILDCARE HOURS END AT 5:00AM BUT, I DO NOT ALWAYS HAVE CHILDREN THAT LATE. SO PLEASE DON’T COUNT ON ME BEING AVAILABLE! PLEASE CHECK WITH ME TO FIND OUT FOR SURE!! PLEASE ALSO KEEP IN MIND THAT EXTRA HOURS OVER YOUR NORMAL WEEKLY HOURS WILL COST YOU $3.00 PER HOUR OR $20.00 PER DAY. I agree to all the terms listed above. I will not hold Munchkins Playhouse or Shelley Drake liable for any accident that may befall my child while in this provider's care. _____________________________________________________________________________________________________________________ PROVIDER RESPONSIBILITY: I will provide a safe, healthy and nurturing environment that supports the physical and emotional needs of the children in my care. Supervised indoor and outdoor play is provided. Healthy nutritious meals and snacks, developmentally appropriate activities and an adequate period of active and quiet play will be provided according to child's age and development. * Will work diligently with family to resolve any type of conflict before terminating care.
