Identification Requirements:
Print the instructions and both sides of the form. Read the instructions. Fill-out both sides of the form and sign it. Photocopy existing snowmobile registration and registrant's drivers license from your state. Send $45 per sled payable to "Hamilton County Clerk". Mail everything to: Dept of Motor Vehicles, County Office Building, PO Box 204 Route 8, Lake Pleasant, NY 12108
