LITTLE LEPRECHAUNS FAMILY DAYCARE CONTRACT AND POLICIES I. FEES Fee Schedule: For the above listed hours of care for _______________, we agree to pay $______. Full payment is required, regardless of whether or not the child is present. Fees are due in advance and payable no later than Friday of the preceding week. A $20.00 fee will be assessed if your fee is not paid on time and your child will not be accepted into day care until your fee is paid. Checks will be accepted. If, however, your check is returned by the bank,, your fee will be accepted in cash only. You will be responsible for any fees assessed to my account. A non-refundable deposit of $_____ is required at the time the contract is signed. This fee holds your child’s spot until his/her start date, and will be used toward your child’s final two weeks of care. II. SCHEDULE The first two weeks of care are probationary for provider, parent, and child. This agreement may be terminated any time during that period. Otherwise, two weeks notice is required if the child is to be permanently removed from this daycare. Except in the case of gross misconduct on the part of either parent or child, provider will tender two weeks notice prior to cessation of care. A charge of $15.00 per fifteen minutes, or any part thereof, will be charged for late pickups. Please make sure you pick up your child by the time stated in this contract. A calendar of special days and holidays is attached to this contract booklet. Full payment is due for all holidays and recesses. Payment is not required during provider’s summer vacation(s). Parent will be given at least two week’s notice prior to all provider vacations, except in the event of an emergency. There is no charge for any other time taken off by provider. Back-up child care must be arranged by the parents. III. MEALS Lunch and two snacks are served each day at no additional charge. Our meal schedule is as follows: AM Snack- 10:00 Lunch- 12:30 PM Snack- 3:15 Should your child arrive later than a meal time, you will be responsible for feeding him/her. Except for special occasions or illness requiring a special diet, please do not send in any food with your child. I am a participant in the Child and Adult Care Food Program and make every attempt to provide nutritious meals for your child. I offer a variety of foods. Your child is encouraged to try new things, but I never force a child to eat. IV. ILLNESS Little Leprechauns is a “Well Child Care Facility”! This means if your child is not feeling well, whatever the reason, you must make other arrangements for their care until they are feeling well and are no longer contagious. Please do not bring your child to day care if they have a fever over 100.0; diarrhea; vomiting; an unidentified rash; or any contagious illness. Please make sure your child is fever-free and symptom-free for AT LEAST 24 hours before attempting to return to daycare. Further, there are times when a child is not that ill, but is terribly uncomfortable and really needs some “one on one” care that I cannot provide and be fair to the other children. At those times I will ask that you keep your child at home. If your child is sick enough to require prescription medication, I require that you keep your child home until they have been on the medication for at least 48 hours and they are no longer contagious. If your child needs to be given medication while at daycare, you MUST write a letter authorizing provider to administer the medication. For prescription medication, provider must also receive a note from your physician authorizing the provider to administer medication. The notes must have: name of meds, amount to be dispensed; time of day to be dispensed; for what condition the meds. Is prescribed for; physicians name, address, and phone number; parents and physicians signatures. The medication MUST be in its original container, which is CLEARLY labeled. If your child becomes ill while at the daycare, you will be notified and must pick up your child IMMEDIATELY. Please follow these guidelines for illness. They are for the protection of all of the children and families. If you are not sure if a child should be brought to daycare, please feel free to call ahead of time. V. DAILY SCHEDULE 9:00 - Free Play, as children arrive 9:30- Circle Time (songs, stories, finger plays, calendar, etc...) 10:00 - Snack Time 10:30 - Art/Craft/Baking/Cooking, etc... 11:00 - Free Play 11:30 - Large Motor Activities (Hopefully, outside play...otherwise, we will dance, play games, etc... indoors) 12:00 - Quiet Activity Time 12:30 - Lunch 1:00 - Story Time 1:15 - Quiet Time 3:15 - Snack Time 4:00 - Outside Play (Free Play or Special Activity, if inclement weather) As you can see, we have a very busy day! Of course, when the weather is really nice, we spend a lot more time outdoors! Please remember to send in appropriate clothing for the season! Snowpants, hats, gloves, boots, and scarves are a daily NECESSITY in the winter, while cool, loose outfits are best in summer. We do have a wading pool and several water toys available in the summer, so please remember a swimsuit everyday. Please also remember to send in a bottle of sunscreen, labeled with your child’s name. VI. PARENT RESPONSIBILITIES Parents are responsible for providing: *Diapers/wipes/special creams or lotions *Two complete changes of clothes (including shirts, pants, underwear, socks), labeled with child’s name or initials, and appropriate for the weather. *Infant formula (bottles should be prepared at home) *Pair of comfortable slippers, to be kept at daycare Please remember to bring your child’s journal everyday! Pick-up time can be pretty hectic, and I may not be able to “re-cap” your child’s day as well as the journal entry! Please do not bring toys from home, except for “lovies” or special toys for naptime. If your child does bring a toy, I will not be responsible for any loss or damage to toy. VII. RATE CHANGES Provider will inform parents of any rate increases at least two weeks in advance. VIII. MISCELLANEOUS Any changes in personal address, phone numbers, employers, emergency contacts, etc... must be given to provider as soon as possible. Names and addresses on emergency form will be kept current. Registration deposit of two week’s fee is required at the time contract is signed. This fee holds your child’s place and is used to cover: violations to contract (late fees not paid, etc...), any damages incurred by your child while in daycare. Otherwise, deposit is used toward your child’s final two weeks of care. IX. TRANSPORTATION AGREEMENT X. IN CASE OF EMERGENCY Parents are responsible for any fees incurred from emergency services. XI. MEDICAL FORMS Medical forms must be kept up-to-date. Please remember to update your child’s medical form annually! Your child must be current with all immunizations before, and during, care.
