Mariner 3
Mariner 3 was launched by NASA on November 5th, 1964.
Mariner 1 and 2 were Venus missions, so Mariner 3 was the first NASA Mars mission.
Mariner 3 was protected by a shield against friction during acent into space. It broke and disturbed the mission.
Another lost mission. Only 3 weeks were left for NASA to send Mariner 3's twin brother, Mariner 4. If they launched it later, the opening window would have been closed, making the mission impossibe.
This was a great task for NASA, the mistake in the building of the heat shield having to be found.
