Phobos 1 + Phobos 2
Phobos 1 and Phobos 2 were two soviet probes that were launched on July 7th 1988. Phobos 1 had the primary goal to explore Mars' bigger moon Phobos, but it also examined the Sun, Mars, the interplanetary medium and gamma ray burst sources. It disappeared because of both human and mechanical errors.
Phobos 2 was sent to examine Phobos. The Soviets were interested in using Phobos as a staging post for a manned exploration of Mars. It made 37 photos before disappearing. Some people say one photo shows an UFO.
