What is a Care Plan?
Definition: A Care Plan describes problems and needs of the elderly. It sets goals for a desired outcome relating to a specific problem; and provides approaches to indicate how tthe Goal will be accomplished.
Content: A Care Plan consists of seven (7) Problem categories: 1. Mobility/Safety/Self-Care 2. Nutrition 3. Elimination 4. Skin Integrity 5. Sensory/Communication 6. Psychosocial/Recreational/Activity/Socialization 7. Medical Management
Seminar content will include:
- Identifying problems/needs or concerns specific to your elderly loved one. - The criteria for establishing goals. - The related approaches for problem-solving. - Identifying resources that can provide assistance with caring for your loved one.
At the end of the seminar, attendees will be able to write an Elder Care Plan for problems,goals and approaches.
Attendees will receive an autographed copy of My Diary: Starter Kit - Resources written by Anise J. Matteson.
Who should attend? The elder, spouses, children of aging parents, non-medical caregivers, family members and friends, Homebound Ministry, Parish Nurse.
