Medical Records


Purpose:  To help clients to set up a health information diary.

Provides help with setting up a medical diary for:  prescriptions, allergies, medications, laboratory tests, x-rays, family history, health care aids, insurance records, policy information,emergency information.

Transition Management

Purpose:  For the elderly who are moving to a retirement com-
                  munity; assisted living facility; or skilled nursing facili-
                  ty.

Provides help with creating:  an Inventory List, Medication Summary, and Photo Album.

Purpose:   To assist the family or responsible party with re-
                  quests for medical information. 

Assist with requests for medical information when transitioning from living at-home; requests for copy of death certificate from the Depart-ment of Public Health; forms completion [for those with difficulty writing.]
