GLSEN Sioux Falls agrees to be included in the group tax return of GLSEN, Inc. as of

June 30, 2001 and declare that under penalty of perjury, that the authorization and information the

Chapter has submitted for the group return (the Annual Accreditation Application) is true and complete.

I understand this means that, if granted by the IRS, the GLSEN Sioux Falls Accredited Chapter

will be included under the 501(c) 3 tax-exempt status of GLSEN, Inc.

To the best of my knowledge, the purposes and activities of GLSEN Sioux Falls is stated

in its articles of organization. Our fiscal year reflects that of GLSEN, Inc.: July 1-June 30.
