We are pleased to announce that MPHA has made huge improvements to our horse show and class entry forms!!!  The gazillion individual class entry forms are a thing of the past... hooray! Below you can download our brand new forms - you will still fill out ONE horse show entry form for each horse (including owner & up to 3 exhibitors). The same back number should be used for ALL exhibitors for the same horse.  Initials of exhibitors and the horse's back number will be used to "fill in the blanks" on the class entry forms, which include spaces for Saturday AND Sunday classes. Classes are divided on separate entry forms for: PHBA Open, PHBA Amateur (+ Novice Amateur + Amateur Select), PHBA Youth (+ Youth Walk-Trot + Novice Youth), and OPEN ALL BREEDS.  Feel free to e-mail us with any questions about our new forms.

**IMPORTANT** **Forms must be filled out separately for EACH HORSE entered **Fill out ALL information requested on each form **Make sure to sign and date all forms before turning in with an open check at the office **See FEES PAGE for explanations of class fees, discounts, etc.
