Fixes for Bookmarks: since Windows uses CRLF pairs, and most Doc files contain
LF only, Bookmarks (which are just offsets into the text) pointed at different
places between DocReader and various Palm readers. Fixed this and made some
cosmetic and other small changes.

caused DB not to install.
