Glory be to You, O Allah, and Yours is the praise and blessed is your Name, and exalted is Your Majesty, and there is no god besides You.

Then recite the opening Surah Al-Fatiha:

Following 'Ameen', you must recite another passage of the Qur'an of your choice. The following chapter (Surah) 'Pure Faith' (al-Ikhlas) is a good short example for Muslims to memorize:

Jalsah

Rise index finger when reciting kalima

Jalsah
In this (jalsah) position, silently say these words (Tashahhud):

Our salutations, our prayers and all pure things are for Allah. Salutation to you, O Prophet and Mercy and Blessing of Allah. Peace be on us and on all pious servants of Allah. I testify that there is no god but Allah and I bear witness that Muhammad is the servant and messenger of Allah.

When you reach the point of giving testimony (when you say "La illaha ilAllahu" (I testify that there is no god but Allah), raise the index finger of the right hand. Drop it back down again upon reaching "ilAllahu" (but Allah).

After reciting tashahhud, you recite Durud or salat an-Nabi, blessing upon Muhammad (salAllahu alayhi wasalam):

O Allah! Let Your Blessings come upon Muhammad and the family of Muhammad as You Blessed Ibrahim and the family of Ibrahim. Verily You are the Pariseworthy Glorious.
O Allah! Confer Your Blessings on Muhammad and the family of Muhammad, as You conferred blessings on Ibrahim and the family of Ibrahim. Verily You are the Praiseworthy, Glorious.

After this, say any du'a from the following:

Allahumma inni zalamtu nafsi zulman kathiran, wa la yaghfirudh-dhunuba illa Anta, faghfir li maghfiratan min 'indika, warhamni, innaka Antal Ghafur-ur Rahim

O Allah! I have considerably wronged myself. There is none to forgive the sins but You. So grant me pardon and have mercy on me. You are the Most Forgiving, the Most Compassionate.
[Sahih al-Bukhari and Muslim]

Rabbij'alni muqimas-salati wamin dhur-riyyati. Rabbana wa taqabbal du'a. Rabban aghfirli wali-walidayya wal-lil-mu'minina yauma yaqumul hisab.

O Lord! Make me and my children steadfast in prayer. Our Lord! Accept the prayer. Our Lord! Forgive me and my parents and the believers on the day of judgement.
[Surah Ibrahim, 14: 40-41]

Allahumma inni a'udhu bika min 'adhabi jahannam, wa min 'adhabil-qabr, wa min fitnatil-mahya wal-mamat, wa min fitnatil-masihid-Dajjal.

O God! I seek Your protection from the torture of hell, and I seek Your protection from the torture of the grave, and I seek refuge with You from the mischief of life and death, and I seek Your protection from the mischief of Dajjal pretending as Messiah.
[Muslim, 588]
