Community Support Services

 

Designed for severely mentally disabled adults who need intensive continuous treatment, these programs provide housing, income, vocational, medical, social, recreational as well as psychiatric services.

Aftercare and Case Management Severely mentally disabled persons are assisted with case management support and management of necessary medication which enables them to live as independently as possible in the community.
Community Treatment Teams Psychiatrists, nurses and case managers provide 24-hour intensive community services to help severely mentally disabled persons live as independently as possible.
Hubbard House A non-traditional approach, this community support program is designed to provide varied services that are consumer driven and range from structured to flexible. Services include: Day Treatment, a Drop-In Center, Day Activities and the Connection, a peer support phone line.
Soaring Sober A unique intensive outpatient service for mentally ill adults with substance abuse problems.
Residential Services Fowler House a residential treatment program for the severely mentally disabled with substance abuse problems.
Norwich House a half-way house for individuals having a history of recurring hospitalization and a need for long-term residential services.
Family Care Program an extended care program utilizing private homes.
Shared Apartments apartment living supervised by a case manager.
Community Residences supported housing within the community.
