| 1st week: Baby comes in for checkup of growth and development. 2nd week: Baby comes in for checkup of growth and development. 2 months: Baby comes in for checkup of growth and development, shots DTAP #1, Polio #1, Hepatitis/Hib #1, Prevnar #1, Rotateq#1 4 months: Baby comes in for checkup of growth and development, shots DTAP #2, Polio #2, Hepatitis/Hib #2, Prevnar #2, Rotateq #2 6 months: Baby comes in for checkup of growth and development, shots DTAP #2, Prevnar #3, Rotateq #3 9 month: Baby comes in for checkup of growth and development. 12 month: Baby comes in for checkup of growth and development, shots MMR #1, Polio #3, Varicella(chicken pox), Prevnar(optional til 15 month), Hepatitis A#1 15 month: Baby comes in for checkup of growth and development, shots DTAP #4, Hepatitis/Hib #3, Prevnar #4 (if not given at 1 year) 18 month: Baby comes in for checkup of growth and development, Hepatitis A #2 2 year old: Baby comes in for checkup of growth and development 3 year old: Child comes in for checkup of growth and development 4 year old: Child comes in for checkup of growth and development 5 year old: Child comes in for Kindergarten physical, shots DTAP #5, Polio #4, MMR #2, Varicella #2 6-12 years: We encourage yearly physicals or sports physicals as needed, HPV 3 shot series after age 9, Menactra vaccine with 7th grade physical. 13-18 years: We encourage yearly physicals or sports physicals as needed Menactra vaccine after age 11 or before college. If your child is coming in for a health care check and is scheduled to receive immunizations, please check with your insurance company to make sure that they are covered under your insurance plan. Not all insurance companies cover immunizations and calling ahead of time could save you quite an expense. |
