Certificate of Error

Applying for a Certificate of Error in Cook County

1) Download Form
a. Go to Assessor’s Website: www.cookcountyassessor.com or call the Taxpayer
      Services Department to order one (312-443-7550)
b. Click on Forms
c. Under “Exemptions” click on Homeowner Exemption Certificate of ErrorApplication
d. Download form, print out, fill out information, and sign.

2)   Verification
a. Make a photocopy of one of the following (if these items contain the property
      address on them): driver’s license/state I.D., voter’s registration card, or land
      locked phone bill
OR
b. Go to Assessor’s Website: www.cookcountyassessor.com
c. Click on Forms
d. Under “General” click on Occupancy Affidavit
e. Download form, print out.
f. Have a neighbor, friend roomate, or relative fill out information. and sign in front             of Notary Public
   AND (FOR PRIOR YEARS WITHOUT AN EXEMPTION HISTORY)
   a. Make a photocopy of the Settlement Statement from your closing
      (i.e. HUD-1, RESPA)

3) Submit
a. Go the County Building: 118 North Clark Street Chicago, IL 60602
b. See the Assessor: Third floor, Room 320 – Exemptions Counter
c. Present form and verification
