What Dr. Hom says about Grand Rounds Case #58 -Periorbital Pain

Case History:
This is a hypothetical case. A 35 year old woman presents from the emergency room with a sore LUL.

Questions:

  1. What imaging tests?

  2. What is the minimum amount of testing to come to a diagnosis?

What Dr. Hom says:

  1. The best imaging study is an MRI to differentiate between an orbital cellulitis from a pre-septal cellulitis from an acute chalazion or hordeolum.

  2. Likewise, palpation would be very helpful. Feel for any hard nodule, but don't press so hard that you might rupture the nodule and create a cellulitis. Body Temperature, extraocular muscle functions are also relevant.
