Visually Significant Pterygium w/Refuseal of Excision

Case History:
This is a 47-year old male who presents from the Emergency Department as a "drop in" patient c/o of a growth over the eye right eye for the past year. He feared that this growth would cause permanent eye damage and wanted a topical drop to remove it. He has been a patient at this clinic for the past year and on each of the previous 3 visits, he had refused excision. He reports no trauma, acute pain, photophobia or sudden loss of vision. He denies any recent history of coughing, sneezing or vigorous rubbing of this eye. . His medical history appears to be unremarkable and apparently non-contributory.

My physical examination reveals that the entrance vision with current ophthalmic correction in this eye was 20/?? The IOPs by App were below 15 in each eye. Dilated fundus examination was unremarkable.

Questions:
1. What do you think the expected best vision is if this was the only abnormality in the physical examination?
2. Would you recommend excision?
3. How would you manage this  patient?

A somewhat dramatic example of a pterygium whose best vision with refraction was 20/125. The patient again refused excision.
