This agreement regards the transfer of a male/female Belgian Tervuren puppy born _____________, sired by Ch. __________________________ out of Ch. _____________________________. This puppy is sold, with AKC limited registration, as a personal companion for the amount of $_________. The breeder and seller of this puppy is Linda Mullen. REPLACEMENT OF PUPPY . The breeder agrees to replace the above puppy with another puppy of like quality from her next available litter should any of the following conditions be diagnosed prior to 36 months of age:1. Hip or elbow dysplasia diagnosed by radiographic evaluation by the Orthopedic Foundation for Animals. 2. Progressive retinal atrophy (PRA), or hereditary cataract (HC), diagnosed by a diplomat of the American College of Veterinary Ophthalmologists. In addition, the breeder agrees to provide a replacement puppy if at any time in its life, the dog develops seizures diagnosed as inherited idiopathic epilepsy by a certified veterinary neurologist. SPAY/NEUTERING REQUIREMENTS : The buyer agrees that the subject dog will not be used for breeding purposes and will be spayed or neutered prior to 12 months of age and that a veterinarian’s certificate of neutering will be furnished to the breeder. If the dog is used for breeding prior to neutering, the above replacement agreement is considered null and void and the breeder is under no obligation to replace the dog or refund any portion of the original price of the dog.NOTE: A REQUEST FOR REPLACEMENT OF THIS DOG FOR ANY REASON WILL NOT BE HONORED UNTIL SAID DOG IS NEUTERED .ROUTINE HEALTH REQUIREMENTS : This puppy has been vaccinated against distemper, hepatitis, bordatella, and parvovirus according to a schedule supplied to the buyer. The buyer agrees to: 2. Vaccinate the puppy against rabies by 4 months of age and annually thereafter. 3. Maintain the puppy on appropriate heartworm prevention all year with annual blood test for presence of heartworms. Your puppy has been started on an approved heartworm preventative. 4. Check the puppy for the presence of intestinal parasites at the time of each scheduled vaccination. 5. Maintain the puppy on appropriate external parasite control such as Advantage, Program, or Frontline. 6. Within 6 months after the dog’s second birthday, x-ray for hip dysplasia and submit xrays to the Orthopedic Foundation for Animals (OFA) for evaluation. The buyer must be notified of the results of the evaluation. ADDITIONAL REQUIREMENTS: 1. This puppy is intended to live as a house-dog and not to be isolated in a traditional kennel setting, yard, or pen. The seller recognizes the need to confine dogs when left alone or for other purposes and urges the use of a crate for those circumstances. 2. The seller strongly suggests enrolling the puppy in a puppy socialization class (sometimes called "kindergarten puppy training" or "super puppy class") prior to 4 months of age and a beginners or basic obedience class prior to 10 months of age. 3. The buyer agrees to maintain regular contact with the seller regarding the health and well-being of the puppy. Both the breeder and the buyer agree to notify each other promptly of any change of address or telephone number so that contact may be maintained. Should the buyer at any time be unable or no longer wish to keep the subject dog, he or she agrees to return the dog to the breeder. The dog is at no time, to be placed with any other individual without the breeder’s permission. REBATES: Because the breeder is committed to furthering the qualities of trainability and versatility, rebates of $60 will be made for any AKC obedience title, tracking title, herding title, or agility title.Signature below indicates that all parties are in total agreement with the requirements of this sales agreement:
