Supplemental Security Income (SSI) is an entitlement provided to families who have children with disabilities and who qualify.  The law acknowledges that these families are financially burdened with additional expenses.  The funds come from the federal government.  The state of New York supplements it, but the funds come together in one check.

As a parent of a child with a disability who lives in New York, you may apply by visiting the social security offices, or by calling the pone number 1 (800) 772 1213.  Ask for them to send you the Medical History and Disability Report - Disabled Child Form.  This form has previously been named the F6 Form, a term that is still used by SSI workers.  The actual number of the form is OMB 0960-0574.  Make sure to simultaneously request a personal interview.  You will need to provide them with:  your child’s social security number, his/her birth certificate, names and addresses of your child’s pediatricians and doctors, educational documents, the Individualized Educational Plan from the board of education and the child’s medical and educational histories, but don’t wait until you have all the documents, since they may help you to collect them.

You will need to personally attend the interview.  The interview should take about one hour.  They will ask you details regarding your child’s educational and medical development.  make sure that you ask details regarding when, how and who is going to notify you about the progress or outcome of your interview.
