                                                          The presently acclaimed model for any study is the problem based model. MD Medicine
                                                          automatically provides us with this template because we directly deal with problems.
                                                          Hence the study is always an active one. One should try to solve a problem first with
                                                          the information one has, also simultaneously noting down the areas of lack of
                                                          information.The holes should be filled with various references.

                                                         To start with, one should be involved with the day to day functioning of the ward and
                                                         patient care.

                                                         The initial acquisition of information and achieving the diagnosis should be based on
                                                         certain formats which are detailed in POMR, Follow up notes and others which will
                                                         be shortly available.
