There is no limit to the number of volumes you would like to request.
1. Name and Address
2. Fansubs you are requesting (include title and volume number).
3. What format you would like the fansubs (DivX, VCD, VHS).
4. How you are paying (money order or PayPal (add an additional $2 to total cost)).

4. Money Order
I will reply to your e-mail with total cost and where to send the payment.
