Though they live in the same houses where they grew up, Jimmy, Dave and Sean have drifted apart over time. Their distance is due to a disturbing and violent episode that occured when they were children. Even now, as adults married with kids, they have never managed to overcome their fear and guilt about what happened. Dave and his wife still live next to Jimmy, who is married to a tough-sexy blonde and has three daughters. When Jimmy's 19-year-old girl is murdered, he turns to Sean, who works as a policeman, and delivers an ultimatum: find the killer fast or I'll go after him myself. Little do they know, the culprit is the last person they'd ever suspect.

- The film was shot in 39 days.
- More than 2,000 people who wanted to be extras in the movie showed up at the Boston Teachers Union on September 14th, 2002 for the open casting call. Carolyn Pickman of Collinge-Pickman Casting told the Boston Globe a week later abotu two-thirds of those trying out would be called to fill a church and a local bar, play spectators at a parade, or walk through other scenes. All types of people from the greater Boston area (and as far away as New Hampshire) attended the call, where they waited in long lines to eventually hand in their applications and get their pictures taken.
- The marching band seen in the movie is the Boston University Marching Band.
- Dennis Lehane, author of the novel "Mystic River", can be seen waving from the back of a convertible in the parade sequence.
- Laurence Fishburn's character, "Whitey", is a reference to the Boston gangster James "Whitey" Bulgar. A "South" and Irish, Bulgar's life was more similar to Sean Penn's role in the movie. However, the irony of naming a black policeman, played by Fishburne, after an infamous criminal nicknamed "Whitey" creates a nice double contrast.

- The tune that the band is playing during the parade at the end of the movie is john Philip Sousa's "Semper Fidelis", which is Latin for "always faithful".
- Winner of two Academy Awards for Best Actor (Sean Penn) and Best Supporting Actor (Tim Robbins). The film was also nominated for four more Academy Awards: Best Picture, Best Director, Best Supporting Actress and Best Adapted Screenplay.
