I am a competent witness over the age of 18 years of age. I personally photocopied the document(s) shown on both sides of this page as proof of service by THE UNITED STATES POSTAL SERVICE that the letter bearing Certified Mail Number _____________________________________ on its face was placed into the mail and was also delivered-to and received-by the Addressee. I have the original U.S.P.S. documents in my possession.

"The receipt for certified mail with post office stamp thereon ... established that mail had been delivered to the post office on that date.... Whether or when the [letter] was deposited in the mail is a question of fact ... and the return receipt is better evidence of the fact than the proof of mailing by the permissive method of affidavit or certification."
