Sailor Moon's first transformation brooch, never given a name so generally referred to as just 'transformation brooch'.
Unlike the rest of her transformation tools, this was not a compact, because there wasn't any crystal to put inside. In both the Manga + the Anime, it was given to her by Luna to make her pay attention to what she was saying.

The Disguise Pen (Dub: 'Luna Pen'). Usagi uses this to disguise herself. In the Anime, the disguises are generally only small - a different hairstyle and a different outfit (only once does her hair actually change colour and her face change). In the Manga, though, her disguises can be much more elaborate, even changing her gender. In the Manga she gets the pen from the Sailor V Video Game by hitting it, hoping that it would spew up another one after it gave out Ami's transformation pen. Only afterwards was it revealed to have magical properties. In the Anime, Luna created it for her so they could pass by a guard station without being recognised.

The Moon Stick (Dub: 'Crescent Moon Wand'). Usagi's first wand, it could be used to heal people and return energy to people who had fainted or died. In the Manga it was given to her by Luna as a symbol that she was their leader. In the Anime it was given to her to return Geisein to a normal human being as well as being a symbol of her leadership. There is a notch in the bottom of the crescent where it will hold the Illusionary Silver Crystal. In the Manga it could also turn into a long staff when it housed the Crystal, which amplified its power. In the Live Action it was her only wand and her primary method of attack - it had both offensive and defensive properties. The Silver Crystal was never housed in it, but the entire golden crescent moon was made of a crystalline substance. When it was smashed, she fainted, just like she did in the Anime whenever the Silver Crystal broke; perhaps the whole wand was made of the Silver Crystal and nobody knew.

The Crystal Star. Usagi's second transformation tool, a compact that held the Illusionary Silver Crystal. In the Manga, she broke her first transformation brooch when she used all her power to defeat Metallia. So she got given a new compact with a space to put the Crystal, so that she could transform and bring her friends back to life. In the Anime, the Cardian Reishi attacked her and smashed her transformation brooch. Usagi detransformed and was sent inside her soul, where she met Queen Serenity. The Silver Crystal appeared and merged with her broken brooch, which turned it into the Crystal Star.

The Spiral Heart Moon Rod. In the Manga, when she met NeoQueen Serenity in the future she gave it to her to help fight off future enemies. Where did she get it from to start with? If we understand that NeoQueen Serenity is Sailor Moon's future self, then she gave herself the wand she got from herself when she met herself again in the future.

In the Anime, it was borne from the love of Usagi + Mamoru. As such, 'Rod Born of Love' has become a meme for a penis (specifically Mamoru's).

Sailor Mercury's first transformation pen. In the Manga she won it as a prize from the Sailor V Video Game (which it is implied that Luna tinkered with). In the Anime, Luna gives it to her when she finds out she's Sailor Mercury to save her from a monster. The coloured end is dark blue. In the Manga, it's a real fountain pen with a removable cap, and she has been shown writing with it. In the Anime it's just a pretty stick.

There, she told her to give it to Rei, who used it to transform for the first time. In the Anime, Luna left it for her when they confronted her at the Hikawa Shrine, suspecting that she might be Sailor Mars. The coloured end is deep red.

In the Anime Luna gives it to her to help Sailor Moon when she is trapped by Geisein, the first Rainbow Crystal Carrier. It has a teal tip (note; not green).

It's revealed in the Sailor V Manga that it was given to her by Artemis, along with her moon compact. It could also be used as a sort of walkie-talkie to communicate with their ambiguous 'Boss' (who was probably Luna). It looked the same as the other three Guardian Soldiers' transformation pens in the Anime, though, with a yellow tip. She showed up posessing it (undoubtedly it was given to her by Artemis in this continuity, too).
