Contract

We/I have received and read the Policy Handbook and will comply with all provisions contained herein, and shall at this time enter into agreement with Missi Hamele of Wee Care Family Daycare for the care of my/our children__________________.

Care is needed for the following days and times:

Summary of important information contained in Policy Handbook:

• Payment is due Friday or the last day of the week of child care services. Payments can be made by cash, check, or money order. There is a $20 fee for returned checks.
  
• A late fee of $10 per day will be charged for payments not received by the close of business Friday.
  
• If you pick up your child more than 15 minutes after your contracted pick-up time, there will be a late fee of $5.00 for every 15 minutes.
  
• In the event that I fail to pay for child care services, and I am turned over to a collection agency for non-payment, I agree to pay any and all costs of collection, including attorney fees and court costs.
