PAPERWORK
FOR ATHLETES
There
are certain forms that all athletes and their parents must fill out prior to
participation. The following is a list of those forms:
1.
Physicals. All athletes must have annual physicals. There are no exceptions
to this rule. Some physicians,
Kaiser physicians in particular, will say that a student does not need a
physical but once every two years. This is not what the CIF and NCS say. We
simply need a note with a signature from your physician stating that the student
can participate in athletics. Physicals are mandatory for all Salesian
students. Check with the front office to see if physicals
are current. Physicals are good for one year (365 days) not just one school
calendar year. A physical obtained in the summer is good for all sports during
the entire school year.
2.
Permission
Slips. All athletes must have
a permission slip signed by their legal guardian. There are no exceptions to
this rule. Any injury requiring a doctor requires a permission slip so that care
can be administered. No permission slip, no care. Do not let this happen. Click

3.
Ejection Policy.
All athletes and coaches must
sign NCS ejection policies. There are no exceptions to this rule. The policies,
once signed, are to be given to the athletic director. Technically, no NCS team
can participate in any contest without verification of the ejection
policy. Click
