Troop 349 Outing Permission Form
I, ______________________________, the parent (or guardian) of
_______________________, give my permission for my son to participate in
the _____________________________________ activity. I realize that the possibility of injury exists and I authorize the adult troop leadership to provide first aid, or to consent on my behalf for medical services they deem necessary. Furthermore, I agree to pay for these services if and when they are provided and hold harmless the troop leadership, the troop sponsor, and the BSA for any injury, loss or damage to personal property that may result on this outing.
By signing this consent form, I certify that I have read and discussed with my son all the pertinent information required for this activity and that he is prepared for this outing.
