- What code name you would like (keep the name
clean)
- Do you have your own equipment? If so, what
(remember, it must have a back sensor).
- How much experience do you have?
- Birthdate, gender
- What town in Maine you live in
- Where you can be contacted (an address or an
E-mail will do)
- Have seen and agreed to the rule (see the Rule
Page)
