There are five catagories of root causes, financial (F), administration (A), communication (C), environment (E), and skill (S).
Financial root cause is the lack of cash flow. It is simply stated as not having the financial resources to fund the solution.
Administrative root causes can be either written or unwritten. These administrative actions may be written, but unenforced. They are major contributors to the culture of the organization.
Creativity includes the generation, storage, and dispersion of information. This includes computers and information technology.
The environment root is represented by things that cannot be changed. This includes but is not limited to economy, technological limitation, or physical constraints.
The skill root is not only the lack of necessary skill, but also the wrong kind of skill
