Kry'san Shelties, Sheila M Coleman (herein after referred to as seller) agrees
to sell to ____________________ (herein after referred to as the buyer) the
Shetland Sheepdog identified below (herein after referred to as the puppy) for
the price of $________ .

The puppy is fully guaranteed for life
against the following hereditary conditions: eye problems, bleeding disorders,
epilepsy, hip dysplasia, congenital heart disease,
and dermatomyositis. This guarantee also covers any
other hereditary problems which, in the opinion of the seller, would make the
puppy unsuitable as a pet. Should any of these problems be diagnosed, the
seller will require a second opinion from a veterinarian of seller's choice in
the buyer's area (cost assumed by buyer). If the diagnoses from both
veterinarians are in agreement, the buyer may return the puppy (it will be
replaced as soon as a suitable pet quality puppy is available), or choose to
keep the dog and have the full purchase price refunded. The buyer agrees to
take the puppy to a veterinarian of their choice within 72 hrs of receiving the
puppy for the above mentioned guarantee to be valid.
The buyer agrees to take the puppy to a
veterinarian for all annual shots, heartworm preventative as instructed by a
veterinarian, annual heartworm test, annual screening for all internal
parasites, and care for any situation which may arise requiring medical
attention.
The buyer agrees to feed the puppy a nutritional
pet food (such as Pedigree, Purina, Pro Plan, Iams,
Science Diet, but not limited to the above mentioned) and supply plenty of
fresh, clean water. The buyer agrees to keep the puppy groomed at least once a
week to remove all mats, tangles, or any other debris in the puppy's coat. The
buyer agrees to keep the puppy, when outside, in a fenced in area which will
prevent the puppy from straying. At no time will the puppy be
outside on a leash, chain, or other restraint device without supervision.
The puppy is being sold as a pet with limited
AKC registration. The buyer agrees the puppy will not be used for breeding for
any reason. The buyer will have the puppy spayed or neutered by the puppy's
first birthday and provide proof of same to the seller.
The buyer agrees to notify the seller if the buyer is no longer able to care
for the puppy and a new home must be found for the puppy. If the buyer has
someone they wish the puppy to go to, upon buyers notification to seller of the
new owners name, address, and telephone number, the puppy may go to that home.
In all other instances the puppy is to be returned to the seller. Under
no circumstances may the puppy be transfer to an individual known to mistreat
animals, a Pound, Humane Society, Animal Shelter, Pet Store, Breeder, Rescue
Organization, an individual associated with a puppy mill, or released into
society without a permanent home.
Buyer agrees the puppy will remain with seller until purchase price is paid in
full.
Buyer and Seller agree to notify each other of any change of address or phone
number.
