Survey: OT Phrases

I am in the process of getting OT phrases translated into various languages. These are phrases I feel would be commonly used with clients when a translator is temporarily unavailable. Therefore, most of these close-ended questions are component-based and designed to be answered with a "yes" or "no." These phrases could be used to set a client at ease, to help take a client history, to form patient checklists and surveys, or during treatment.
I am not sure which phrases to have translated--I need your help and expertise. After viewing the following phrases, please checkmark ONLY the ones you feel should be translated. There will also be a blank line underneath each phrase, so if there is a better way to say it, please type it in AND checkmark it.
Also, if you are a fluent (preferably native) speaker of another language or have software capable of writing in native script, and would like to get involved in this project, let me know.
