Be sure to read the article explaining Prior Notice before attempting to use this form. Parents should understand its role during the IEP meeting and just what its purpose is as defined in IDEA. Below is a sample for parents to print out and use at each IEP meeting. It is a way to get into the record all proposals put on the table for discussion during a meeting.  By recording each proposal parents can then track the disposition of each one by the end of the meeting. If a decision cannot be finalized at the meeting, use the last two columns to track who is in charge of following up and when they are to report to the team on a decision.
