Oregon Vital Records

Vital Records staff will not be able to release preadoption records at the public service counter in Portland. It may take several days to begin getting sealed files from the State Archives which are stored off-site. Adoptees over the age of 21 may place orders for the records in person, or order by telephone (503/731-4108) or mail.

HOW SHOULD APPLICANTS ORDER?

Applicants who request records should clearly identify themselves as an "ADOPTEE SEEKING A PRE-ADOPTION BIRTH RECORD." Without this information we will send the current legal record. Enclose a $15 non-refundable search fee payable to Oregon Health Division. Include your current legal birth record information:

Full legal name (after adoption)
Date of birth
Place of birth
Adopted mother's full maiden name
Adopted father's full name

Each order should show the adoptee's mailing address and daytime telephone number. Our office will call if we find the applicant has not given us correct and complete record information that matches the legal birth certificate. Pre-adoption birth records may only be ordered by the adoptee and will only be mailed to the adoptee or their legal representative. Attorneys should include a written statement of whom they represent and the current legal birth record information listed above. Anyone else ordering needs a permission note with a notarized signature of the adoptee to show they are representing the adoptee.

Also Oregon has pretty good indexes, Death by name in 5-10 year bunches, Marriage by Groom's name as well as by Bride's name, Divorce by Groom, birth indexes are sealed for 100 years.
