REQUEST FOR PARENT PERMISSION FOR SPECIAL SCOUTING ACTIVITY

What: Union Canal District Spring Camporee.

Date: May 4-6, 2007

Place: Pine Grove State Park.

Meeting place and time: Church @ 5:30PM (Friday 4 May. 2007

Approx. time and place of return: 11:30 AM Sunday 6 May 2007. At the school parking lot.

Cost: $22.00, payment due 30 April 2007.

I case of an emergency, the leader will immediately notify the parents.

Place/Time of Departure: Church @ 5:30 PM Friday (4 May 2007)

Place/Time of return: Approximately 11:30 AM, Sunday 6 May 2007.at the school parking lot.

I hereby relieve the Boy Scouts of America, Troop 42, the sponsor, and the adult leaders from any responsibility In the case an accident might occur.

In the event of illness or accident during the course of this activity, I request that measures be instituted without delay as the judgment of the adult leaders and/or medical personnel dictates.
