HEALTH INSURANCE:
HEALTH MAINTENANCE ORGANIZATIONS (HMO's)*
An organization that provides a wide range of comprehensive health care services through a designated group, or network of doctors, hospitals, labs and other providers. To receive benefits, you must see the doctor you select as your primary care physician first for care or a referral, except in the case of an emergency. Your choice of
doctors is restricted to those in the network.
PREFERRED PROVIDER ORGANIZATIONS (PPOs)*
Health care providers who are under contract to provide care at discounted or fixed fees. Unlike HMOs, health plans with a PPO allow you to choose any doctor at any time. However, if you select a non-PPO provider you will pay more out of pocket for services than you would if you selected a PPO "network" provider.
POINT-OF-SERVICE (POS)*
A health care plan that offers the employee the flexibility to choose how to receive health care services at the point of service. Services are covered at two levels: in network and out-of-network. In network provides coverage similar to standard HMO coverage, featuring comprehensive health care services with low out-of-pocket costs. Out-of-network benefits typically have limited coverage and higher out-of-pocket costs.

What is HEALTHY FAMILIES?
The HEALTHY FAMILIES PROGRAM helps parents get low cost
Medical Dental Vision Insurance for their children.
Who qualifies?
Children ages 0 to 19 that don’t have insurance and are California residents.
Can I choose my doctor?
Yes, if your doctor is a participating provider in the Healthy Families Program.
It’s very easy to qualify, just have the following information available:
·Child’s Social Security.
·Child’s birth certificate, hospital certificate, Green Card, or Passport.
·Proof of pregnancy by a doctor, if pregnant.
·Family’s Proof of income for all working parents
Pay-Stubs Letter, or Tax return).
