I understand that if for any reason I cannot keep this dog I am required to
return the dog to Sheltie Rescue

There is a $150.00 adoption fee to help offset rescue expenses. A home
visit to assess pet compatibility will be part of the adoption process. In
consideration of SASR allowing me to adopt ________________as my pet, I
promise and agree to hold harmless SASR, its members and volunteers from
any and all claims of liability for the conduct of this dog on or after the
date of this adoption. The undersigned further agrees and intends that this
release of liability and indemnification shall apply to all known, unknown
and unanticipated damages resulting from adoption, ownership, and control
of such animal. I will assume all responsibility for the dogs medical and
daily care from this day forward.

All of the answers I have given are true and correct to the best of my
knowledge.
