The "Follower" pictured above is a fan-made replica designed from the actual wooden ones that were used in the film.
Mark Worley, a replica creator, has this to say about his prop:
"When you watch the movie, the best close-up of the prop is after a runner is terminated and Logan requests an identity check - he brings the Follower down to compare his on-screen image with the dead runner. This is the best close-up of the follower, and it has the wrap-around, smaller view screen. I suggested to the man I was working for to match the close-up version of the Follower as the fans will no doubt compare it to this scene. The Follower in this scene was the only "working" Follower. If you watch carefully in the editing of the scene, Michael York actually starts to put the Follower back on his belt. Not towards the runner. The close-up shot was an after thought in the production of the movie, and a working Follower was added. Problem was the existing design did not support the easiset way of creating a working effect - that of simply back-lighting a slide of an actor. The view-screens as created in the dummy Followers were much bigger than a slide, so they hollowed out a Follower, made a wrap around plate with a hole in it just big enough to hold a slide, and back-lit it - VIOLA - finished effect. That's why my replica looks like it does."
Also, the dummy props had a strip of velcro accross the top-front of the prop (above the view screen) that was stapled / nailed to them for the purpose of attaching it to the costume. (Your original prop picture does not have the velcro, but you can see the holes where nails were put in.)"
