Let's start with some of the more intriguing disorders/cases:
in Psychiatry
:
Paraphilias
Exhibitionism (exposure of genitals to strangers), Fetishism (finding nonliving
objects erotic eg women's underwear), Paedophilia, Frotteurism (fantasies, urges
or behviour centred around rubbing self against non-consenting other), Sexual
masochism and sadism, Voyeurism (fantasies, urges or behviour centred around
watching non-consenting others undressing, or having sex).

in Trauma:
Self inflicted injuries are always the most disturbing.
Here's a case where the patient stabbed a radio antenna into his heart (source
trauma.org).
To the left is the chest Xray where you can clearly see the antenna in the heart and, to the right, the "real" view.
Seatbelts are supposed to save your life (and they do), but they're not as harmless as they seem. In impact, a seatbelt can do a lot of damage (but at least they keep us alive. bruised, but alive).
In dermatology: Dermatology surprisingly cover some of the more gruesome cases. If you were feeling nauseous already, don't scroll down.
Most
disorders/diseases get worse if they are not properly treated and skin disorders
are no different. It's surprising to see how long
people will wait to see a doctor and how far they'll allow their disorders to
evolve.
Chromomycosis is an infection of the subcutaneous tissues, sinuses, brain, and other tissues caused by dark, melanin-pigmented fungi. Most infections start on the arms or legs with small, itchy, enlarging papules. Unfortunately, some people let it get to this stage (picture on the left).
