USA BADMINTON TOURNAMENT RELEASE FORM
Event: New England Junior Open Date: May 14 - 16, 1999
Organizer: Gut 'n Feathers Club, Marblehead, MA
In consideration of being allowed to participate in the above USA Badminton sanctioned tournament or event, the undersigned:

1. Agrees that prior to participating, the undersigned or his designee will inspect and review the facilities and equipment to be used, and if he/she believes anything to be unsafe, he/she will advise the organizer(s) and refuse to participate in the tournament/event.

2. Acknowledges and fully understands that each participant will be engaging in activities that involve a risk of serious injury, including permanent disability and death, and severe social and economic losses which may result from their own actions or negligence or from the actions or negligence of others, the rules of play, or the condition of the premises or any of the equipment used. Further, there may be risks not reasonably foreseeable at this time.

3. Assumes all of the above mentioned risk and accepts personal responsibility for the damages following such injury, permanent disability, or death.

4. Releases, waives, discharges and covenants not to sue USA Badminton, its affiliated clubs, their respective administrators, directors, agents, coaches , and other employees of the organization(s), other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and leasors of premises used to conduct the event, all of which hereinafter referred to as ÒreleaseesÓ, from any and all liability to the undersigned, his/her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused, in whole or in part, by the negligence of the releasees or otherwise.

The undersigned has read the above waiver and release, and signs it understanding that he/she has given up substantial rights by doing so.
