CASE 3

28 - year - old man presents with symptoms of malaise, lethargy and mild upper abdominal pain. Past history reveled that he had received blood transfusion for traumatic accident 6 months ago. He is sexually active only with one partner for the past 5 years. On physical examination, he is afebrile, non-icteric, no lymphadenopathy and mild tender hepatomegaly with smooth border. Laboratory studies show white blood cell count 14000, atypical lymphocytosis, normal total bilirubin, serum alanine amonotransferase (ALT) 1100 U/L, serum aspartate aminotarnsferase (AST) 600 U/L, and serum alkaline phosphatase 100 U/L.
