REQUIREMENTS IN STARTING A BUSINESS
I.  DTI - Registration of Business Name
1.  Single Proprietorship (Requirements)
- Applicants must be 18 years of age and above
- Filipino citizen
- 3 pcs. 2" x 2" ID picture
- preferred business name  & 3 other alternative business names
- Registation fee of P 300.00
- 3 pcs. Documentary stamps
- 2 machine copy of either voters ID/birth certificate/PRC license
if surname is alien sounding
2.  Corporation/Partnership (Optional)
- 2 sets  certified true copy of SEC Registation, by laws &
Articles of Incorporation
- 3 pcs. Documentary stamp
- Registration fee P 500.00
- 2 machine copy of Board Resolution authorizing signatory
II.  Mayor's Permit (License to Operate)
-  Business Name Registation Certificate
- Brgy. Clearance - 3 copies
- Residence Certificate
- 2" x 2" ID picture
- Folder (long white)
- Fire Certificate - Bureau of Fire Department
- Zoning Compliance
- Tourism Registration-Memo Circular No. 147, series of
1993 of the DILG to include the business establishments SSS ID
number of employee
- Food Handler's Permit - for establishment serving foods
III.  Bureau of International Revenue
- T.R.U. (Taxpayers Record Update) forms available at BIR office
- Mayor's permit
- DTI Business Name Registration Certificate
- Tax Identification Number (T.I.N)
- Applicant's Birth Certificate.  If with dependents, submit also
their birth Certificates
- Marraige contract for married applicants
