The Lexciantos Campaign.

It was a series of blunders that brought this thing to pass. All caused by the insanity of one Eldar, who would never know the havoc he would cause.
Lexciantos, a simple Imperial system. Rich in agriculture, on the far edge of Imperial space.
The movement of a set of Eldar ships into the system would go unnoticed by the local populace, but the Astropaths would notice the scream, that split the mind of any who listened in two. All contact with terra failed as the planets Astropaths minds failed. For the navigator of the Eldar fleet had gone insane, and thus stranded the Eldar in amongst the asteroid belt, their craft crashing into the unforgiving rock.
On Lexciantos 2, seat of the Imperial governor for the system panic reigned, until Governor Luther Halathas decided on what must have occurred, clearly the Imperium has fallen, and in that fire he must rise to become the new Emperor! He would bring order to his system and rule as their King! He swiftly removed those amongst the local Imperial guard company who would not follow him and established his new empire. The people, having never seen terra, knowing only legends, were happy to believe in his legend.
And thus was the heresy formed.
Meanwhile the Eldar scavenged what they could from the ruined crafts, and over the next few years built small craft in preparation of exploring the local system (still unaware of the local human populace) in search of a planet to survive on longer, until their craftworld could be contacted.
From The Imperial records.

The Lexciantos System is a single star system on the edge of Imperial space. Orbited by six planets and containing an asteroid belt that cuts off the first two planets from the rest of the system the Lexciantos Astropaths have long been responsible for guiding many food shipments back to the Imperium.
Current System Governor: Luther Halathas.

Lexciantos 1:
The Factory world. Lexciantos 1 is a heavily industrialised world, covered in factories and buildings this world's surface is ravaged by the heat from the nearby sun and was unusable for agriculture.

Lexciantos 2: The system Government world. A rich Agri-world famed for the large quantities of food it ships back to the Imperium.

The Lexciantos Asteroid Belt: Of little mineral importance this Asteroid belt makes travel through the system dangerous.

Lexciantos 3: The Jungle. A planet covered by thick and dangerous jungle, containing some of the most hostile plant life and animals in the sector. Also used as a training base by the local Imperial Guard battalion: The Lexciantos Swamp-Cats (named after a local beast).

Lexciantos 4: The Waste world. Once a heavily industrialised planet akin to Lexciantos 1, this world was ravaged by a series of volcanic eruptions and now is covered in ash wastes.

Lexciantos 5: The Harsh Agri-world. A fertile world marked by its inhospitable climate, Heavy rainfall and cold weather make this a place populated by only a few rugged settlers.

Lexciantos 6: The frozen world. Its distance from the sun makes this world a cold frozen tundra, designated as unfit for human occupation this world has never been settled.
