This is basically a registration form that you will send via fax to assure your participation in the grand event.

After filling in the pertinent company information, a section at the bottom gives you three options to choose from, regarding your participation:

Yes, we will attend the Awards Ceremony on August 21 and are paying the $8,000.00 (Eight Thousand Pesos) plus 15% VAT by means of a bank draft;

Yes we will attend the Awards Ceremony on August 21 and are paying $4,000.00 (Four Thousand Pesos) plus 15% VAT by means of a bank draft and we agree that we owe and shall pay the balance upon registering for the Awards Ceremony

We will not be attending the Awards Ceremony. Please send the trophy and all documentation to our company address. We are attaching a bank draft for $8,000.00 (Eight Thousand Pesos) plus 15% VAT.

At the bottom of the form is the following small text:
During the Awards Ceremony a representative of each company may speak for 30 seconds.
