2. You do not have to have a site, but you must have a valid e-mail address. If you do have a site, you need to put up a code with a link back to this site, either text or image, prior to submitting. If I check your site and the code is not there, I will only list your e-mail address.
3. If you have a site, I will not tolerate pornography/distasteful nudity, illegal activity or any form of hate.
4. When filling out the form, do not use shortforms like "D," long aliases or initials. Just a simple first name is fine, or a short alias is acceptable. If you live in the UK, please specify where.
